I always thought he was crazy and the girl was dead. He's waiting around for her night after night and she's never coming back. "Can't leave if I wanted to". Some of the ghostly imagery in the video, phones covered in dust, her body swaying there like she hung herself... all goes a long way to further this impression.
When I first heard this song I was in high school, we had just studied some Keats poem or something, about a guy who killed his wife to keep her near him, or something morbid along those lines. Would definitely fit the Queensryche vibe.

The video does give rather dark and morbid imagery suggesting perhaps a darker meaning, such as yes the girlfriend or wife has committed suicide or has died -- or even darker perhaps, a crime of passion like she cheated and he killed her. It's possible the video was created that way to show the possible many meaning/ layers in this song. It is an amazing song about what passion and love can drive us...

Plain as day.... This song is about his wife or gf always gone, never home. And he misses her. He wants her back at home cookin for him and spending time with him.
The lyrics to me sound like a long-distance relationship, and the guy wishes the distance wasn't so long.
Hear me out. I'm getting a different vibe from this song. I think they had an argument because she's having an affair. She's left, and he's waiting for her to come home. She might not come home, though: she's with her other man.
Oh, and this is not the first time it's happened, either: the bridge fully implies that this happens regularly: "I know I said it before/I hate the sound of the closing door/and your footsteps walking away."
It's kind of sad: this time, she might not come back, and he's alone at home waiting for her.
