To me, this seems like having to get a surgery you could have avoided by just taking better care of your body.

"Baby, how'd you get so cold? Major sympathy and surgery" seems like someone acting upset that the narrator put themself in the hospital, but because of how much they care.
Possibly something involved with overdrinking because "my head is a gun" sounds like a hangover.
