It means she is the woman he is with now but he is still in love with the woman from his past....He'ss till holding on to his past love and all his current lover wants is to make there love last and be strong.

@terribrooke03 you are totally right,but I just wanted to translate the last stanza of the song:) The girl that the narrator is addressing obviously is well known for being with multiple men.The narrator knows this.She also knows that this particular girl doesn't love all of these men-and the "fool" that she's in love with is no exception.The narrator still loves said "fool" though and it breaks her heart "in two" that the sleeper-arounder leads the "fool" on like she does.
