I think much like another song “Anti-Matter” (that's also on the same album as this song), this one is also is inspired by a horrifying van crash the band experienced on Nov 3, 2022. This, much like the other track, sounds like it's an extension what they shared while huddled in the wreckage, as they helped frontman Garrett Russell stem the bleeding from his head wound while he was under the temporary effects of a concussion. The track speaks of where the mind goes at the most desperate & desolate of times, when it just about slips away to all but disconnect itself, and the aftermath.

This song is written as the perspective of the boys in the street, as a whole, and what path they are going to choose as they get older and grow into men. (This is why the music video takes place in an orphanage.) The seen, and unseen collective suffering is imbedded in the boys’ mind, consciously or subconsciously, and is haunting them. Which path will the boys choose? Issac Hayes is the voice of reason, maybe God, the angel on his shoulder, or the voice of his forefathers from beyond the grave who can see the big picture and are pleading with the boys not to continue the violence and pattern of killing their brothers, but to rise above. The most beautiful song and has so many levels. Racism towards African Americans in America would not exist if everyone sat down and listened to this song and understood the history behind the words. The power, fear, pleading in RZA and Ghostface voices are genuine and powerful. Issac Hayes’ strong voice makes the perfect strong father figure, who is possibly from beyond the grave.

Every time I listen to it.. it makes me think that the mouth he's talking about isn't on her face, if you catch my drift. He's too lazy to go down on her, and he's 'already gone'. "Hey you know me, and I know you." And then he calls her a 'slag' which is apparently british slang for a slut.
It could be all a big dirty joke, or a commentary on the whole male chauvinist / selfish asshole manly douche thing, or he really just hates going down on women/her.. and if he's already gotten his fill then she's a 'slag' for asking for it. It's like.. 'Oh c'mon baby, your pussy is a mile away, don't be such a slut.' And then 'Hey, you know me', as if that validates his attitude, 'and I know you', (you're a slag, lol) more excuses.
If there's anything to interpret from the tone of the song, it's that he's pretty angry.. but that sense of humor is always hiding in the shadows somewhere. Honestly, I'm glad that the intent of it (and so many other Pixies songs) is left a mystery. Ambiguity is what allows insanely awesome songs like this mean so many things to so many people. =)

After hearing numerous live clips of this song, I think the lyrics are way wrong. Here's what I got out it says
Your mouth's a mile away(x8) Your mouth's (argh!) all the way gone Hey, you know me Hey, can't you see? (not "encatuse", whatever the hell that means, and the correct spanish is "me conoces".)
Your mouth's... silly (not "slag").
That IMO, makes more lyrical sense. What it means? Well... I think it involves the incredulity of a relationship. WOuldn't be surprised if it's tied up in some sexual way.
Either way, this song is friggin' awesome. The energy of it is enough to blast you off your shoes.

I believe the overall theme to the Pixies song, "Rock Music" is either referring to drug addiction or someone leaving a bad/violent relationship.
"Your mouth's a mile away" is just another way of saying "no matter what you say, I'm not listening to you" This could be a reference to someone trying to lend advice to a drug addict or possibly a girlfriend who is trying to reconcile with her bitter lover, before he leaves permanently. Again, no matter how much this person speaks, it does no good. The drug addict (or the heartbroken lover) has made up his mind. The other person's mouth might as well be "a mile away".
The next verse, "I'm already gone" hammers this song's meaning into the listener's skull, yet again. It simply restates that its too late. The drug addict (or the lover) has made up his mind. "Gone" means that the addict considers himself as good as dead or figures he will be dead soon, due to the life hes living. In the lover's case, "gone" would refer to literally moving away or leaving.
Correction I just wanted to make it clear that the actual lyrics to the next verse are "Hey you know me, Me conoce ". You can find these and all other official Pixies song lyrics @ pixiesmusic.com. In this line the meaning is exactly parallel with how the lyric reads. In other words, the singer is saying "You know how I feel and what I'm all about, so why are you so shocked?"
