I believe the overall theme to the Pixies song, "Rock Music" is either referring to drug addiction or someone leaving a bad/violent relationship.

"Your mouth's a mile away" is just another way of saying "no matter what you say, I'm not listening to you" This could be a reference to someone trying to lend advice to a drug addict or possibly a girlfriend who is trying to reconcile with her bitter lover, before he leaves permanently. Again, no matter how much this person speaks, it does no good. The drug addict (or the heartbroken lover) has made up his mind. The other person's mouth might as well be "a mile away".

The next verse, "I'm already gone" hammers this song's meaning into the listener's skull, yet again. It simply restates that its too late. The drug addict (or the lover) has made up his mind. "Gone" means that the addict considers himself as good as dead or figures he will be dead soon, due to the life hes living. In the lover's case, "gone" would refer to literally moving away or leaving.

Correction I just wanted to make it clear that the actual lyrics to the next verse are "Hey you know me, Me conoce ". You can find these and all other official Pixies song lyrics @ www.pixiesmusic.com. In this line the meaning is exactly parallel with how the lyric reads. In other words, the singer is saying "You know how I feel and what I'm all about, so why are you so shocked?"
