I've read on forums that it's maybe about an alien abduction. Apparently she's said that at concerts. Maybe not alien abduction in particular, but abduction. Which of course could translate to insanity, or corruption from fame, or kidnapping...
Death. This seems to the what this song is about. A formerly active person, with family portraits around her, she abruptly recognizes that death is near. She wonders why she didn't see it coming, but nevertheless welcomes it.
An incredible song. One of her best.
