Yeah Biko refers to the Nigerian word, i think the nearest english equivalent would be something like 'dear'. like people have said, its a term of affectionate greeting used by and to loved ones, and used by his parents. Personally I think this song is about someone possibly dying that Kele is trying to help but can't. I don't think that the "cancer" mentioned is literal though.
