I am fairly certain that this song is about the road to recovery from a bad drug habit like the one that Anselmo experienced. In Headbanger's Ball, he said that this was the first album he did "bone sober". The "frustration/out-patient/to himself/navigate slowly/a step at a time/ towards the law" really makes me think about recovery from heroin.
