its about giving up on trying to get his ex back.
Well not only does it seem to be about giving up on his ex, because the way it sounds, he hasn't given up. He just can't get over her and it's putting him in a depression where he doesn't care about anything or anyone, including himself anymore.
Most people that end up in a state of depression, don't want to look up (holding your head up) because they know they'll get shot down everytime. (is hard when you just want to stay on the ground)
It's pretty much saying he's not content with the way things are, but he'd rather be hurting on the "ground" than getting his hopes up and being shot down again.
I think that Lespaulsmith is right. But there's more to it than that. The entire album is a concept on a journey from a suicide to the afterlife. This is the time in between him killing himself and making it up.
"I started feeling bad for myself today". This is him being hit by all the bad stuff that happened in his life. "But then I stopped 'cause I don't care". All that stuff leads him into the suicide, and him ignoring everything else.
"About fifty years left of just waiting around". This is saying that he still has 50 years of life left, but to him, it's going to be nothing more than waiting for the end. And, even though someone out there might love him, he's still going to end it all. ("Put your heart on your sleeve and watch me pull it down.")
I think this is the narrator's denial with everything after his suicide. He regretted what he did and now he's denying everything even his feelings.
