This song holds personal meaning with me, and I can relate to it's meaning. I'll explain it through my own experiences and the context I see it in. . .
The narrator was once in love with the subject. The subject meant everything to the narrator, and the narrator was eventually wronged beyond belief. He tries to move on, but he never can because 'underneath it all' the subject still remains. He's willing to do anything to shake the scent of heartbreak, but he never can. He just wants to be able to let go, but it's impossible for him. He tries, and tries, and only defeats himself more and more. Perhaps the only ending for this torment he's experiencing is suicide.

Yes, it hurts when somebody you really love hurts you that much, but it hurts even more when you care so much that even after everything they.ve done to you, no matter how horrible, you still want them back, but cant have them.

How is it silly to think it might be about God? Trent was raised in a Christian household and even flirted with Christianity at one point in his younger years. If you've ever had any sort of relationship/understanding with the Lord you'd know it's not something you can totally blank out of your thought process. No matter if you are angry at him or choose to ignore/not believe him Him. The Fragile is a really introspective album that I feel touches on a lot of Trent's softer more vulnerable emotions. During the writing process when Trent was really searching deep he may have unearthed his connection with God and thus we have what's "underneath it all". Underneath all of the hardness, despair, and self-searching for all these years is it somehow laughable to assume that Trent may have had a weak moment wherein he breifly heard God's call?

This is one of the songs Trent dedicated to his grandmother after her passing. Rather than possibly being about God, I would venture to guess it is about her. Additionally, this song has never been performed live, which could lend to it being quite an emotional track for Trent.
Underneath it all, Trent still knows that God is true. He tries to become numb to the fact and ignore the feellings but they're still inside. He kills his brain, perhaps with drugs or just trying to kill any though relating to a true God.

@DJgif I think for some people it's harder to deny God. I was an atheist for the first 20 years of my life and I'd spend my days blissfully denying God's existence in discussions with other atheist friends but when I'd go home at night and lay awake in bed I just knew that I was wrong and it scared me. I KNEW there had to be something more. It was like I was in The Matrix or something. Friends I mentioned it to told me that I was weak and that they didn't need God as a crutch to...

I agree with Wounded Fate. I'm sorry but not every song can be about some lover who hurt Trent or some relationship that went sour. If that's the case music certaintly lacks any depth whatsoever. It seems to me that most people seem to think that nearly every song has to do with said stuff.
Yeah, the first three posts have it I think. I'm not sure Trent has anything good to say about trhe man upstairs. Its about someone who "still remains" in him, even though he's tried everything to attempt to erase them from his head.
Perhaps what remains might be a desire to kill himself; If there is a cohesive storyline to the albums, maybe it's left over from The Downward Spiral if the character never actually killed himself. He's tried to do many things to numb the pain, possibly even drugs ("Kill my brain"), but nothing can make it stop.
