In regards to the meaning of this song:
Before a live performance on the EP Five Stories Falling, Geoff states “It’s about the last time I went to visit my grandmother in Columbus, and I saw that she was dying and it was the last time I was going to see her. It is about realizing how young you are, but how quickly you can go.”
That’s the thing about Geoff and his sublime poetry, you think it’s about one thing, but really it’s about something entirely different. But the lyrics are still universal and omnipresent, ubiquitous, even. So relatable. That’s one thing I love about this band. I also love their live performances, raw energy and Geoff’s beautiful, imperfectly perfect vocals. His voice soothes my aching soul.

This is a hauntingly beautiful song about introspection, specifically about looking back at a relationship that started bad and ended so poorly, that the narrator wants to go back to the very beginning and tell himself to not even travel down that road. I believe that the relationship started poorly because of the lines:
"Take me back to the night we met:When the night was full of terrors: And your eyes were filled with tears: When you had not touched me yet"
So, the first night was not a great start, but the narrator pursued the relationship and eventually both overcame the rough start to fall in love with each other:

Like many relationships that turn sour, it was not a quick decline, but a gradual one where the narrator and their partner fall out of love and gradually grow apart

Okay, this is one hell of a long-shot but this is what I've got from it. Forewarning, I'm a bit of a nerd.
This strikes me as a letter to his unconscious mind from his conscious mind. Or if you prefer, to his id from his ego.
"I know you well, you are a part of me. I know you better than I know myself."
He sees his desires and their tactics/trends (id) consciously but does not understand the processes they take to reach conscious actions.
But he also knows consciously that they are a part of him, and as a result his. In that sense, this song is simply a threat to himself that he's moving inward and making progress in converting subconscious (id) to conscious (ego).

Judgment and speech are in the ego, or conscious mind. The unconscious can't be eliminated or even really excavated, but it controls via fear and other emotions and in knowing that it can't really hurt you, it can't really control you.

Seems to describe personal growth by means of ego death.

This last instance, however, is different. It's acknowledgement, and acceptance. Finality through sacrifice of ego.

First of all, I don't give a damn what MJK wrote this about. It's how WE interpret it in our OWN PERSONAL way that matters. So, even if Maynard did write it about his penis, it still seems to me like he's referring to his friends... We're a part of him because Tool wouldn't be anything if it didn't have fans. And I think he's saying we're all mindless droning TOOLS because we think it's about his penis, and we've become such TOOLS that if Maynard served us a platter of his shit on CD, we'd eat it right up. :)
Anyway, that's how I like to see it. I'm sure it's got holes in it though; there's no reason to point them out to me. H's explanation was pretty valid too.

Looking simply at the lyrics and not listening to the song i could see how you would think this is about Maynard's penis or about masturbation. But he sing the song with such anger sometimes and passion i really dont think its about either of them. I think Maynard is just messing with others ;)
I think its about defeating fear. Something which is a part of you, but not you. I personally think its something to do with having to know your enemy, the enemy being evil and having to become it in order to defeat it. If you do something evil you learn how it works and how to defend yourself from it in the future.
