To me, it seems like Manson is/was in a relationship or good friendship with someone, and there was a discrepancy in the relationship about the morality of ingesting child-sourced adrenochrome, for which I think 'sunsets' in this song is a codeword, and the hard metal is the needle. It sounds like one of them is scarred by the practice (terror in the teardrop), and possibly that they need other drugs to cope with the reality ("make it snow"). "No matter how cold you are" either means the person is the evil one, or they have gone cold in the relationship after finding out the truth. The 'smile like a rifle' probably means they try to cope/pretend with a smile, but there is the very real threat of them 'triggering' to do something, like go out and tell people the truth.
"Cast out now, no fire to smell" probably means a person has been banned from the Saturn religion, and can no longer be a part of the cultish practices.
The rest of the song seems to talk about the world ending or changing drastically, which has links with this elite circle.
