Again, self-explanatory - Tonight is the last night you'll ever see. Maybe this is relating to the "stars flash before my eyes". Perhaps the author is watching the night-sky on his very last night alive.

The author is slowly sinking into his very last sleep.
"Oh my god so many people leaving me behind"
The author dies and every1 else keeps living - they are living him behind.

I'm actually not sure about this line. Perhaps while dying, the author is being revealed all the things he or some other person has tried to hide.
On a personal note, this was the first song that got me into Tesseract, and I immediately fell in love with them. So mystical and deep, yet groovy and heavy and fat in sound, like no other band. I was going through some life crisis and personal transformation while I discovered this song, and it helped me transcend everything and become a better version of myself. Pure meditative song!
