He's talking about us lucky folks who can afford the ordinary miracles of medicine that get so many out of trouble. The people he probably envied. He died because he couldn't get insurance because of his condition, which was partial paralysis and complications. He probably felt very alone. He had lots of friends and fans but not enough money for his astronomical medical bills.
