I see the song's meaning as a general view of the world as well the singer's self:

"I'm gazing upward, a world I can't embrace There's only thorns and splinters, venom in my veins It's okay to cry out, when it's driving you insane But somehow someday, I'll have to face the pain"

They've been so hurt by people in the world, made fun of, attacked, insulted, whatever happened to them caused them so much pain to the point that they no longer see themselves as apart of the world that they live in. They're constantly enduring tons of pain, but they feel like, one day, they'll just have to deal with it, unfortunately.

"It's all gone cold But no one wants the blame It's all so wrong But who am I, who am I to say?"

I feel like this is a pretty important part of the song; they're saying that, although the world is very dark, hurtful, and cold, they're also a part of it. Though the singer is complaining about the people around them, they too realize that they are also a person at the end of the day. So they feel like there's nothing they really have to say in the matter (which I feel like shows ultimate humbleness).

"My heart's an endless winter filled with rage I'm looking forward to forgetting yesterday"

The singer's incredibly infuriated and perhaps turns to alcohol (I know Ivan Moody [5FDP singer] did this for a while to forget the pain he was enduring).

So basically, I think that the song's about a person living in a world that leaves them cold; they don't want to be a part of it, it's a terrible, awful place filled with pain and suffering caused by people, but then again, at the end of it all, they, too, are a person, and realize that there's nothing they can really say about it.
