This song is obviously about a guy who had a girl, and she was murdered. The plot twist was he murdered her, but it was his other personalitiy, as he has multiple/split personality disorder.

It might be exactly what it says,I'm not 100% sure on this one, but I'm my option I think the "dreaming again" might be when he's in his other personality, meaning he might think he's dreaming when he's the other personality.
"But don't believe what he says, he's not real" and it goes on to say "he's murderous" or "he's a murder"
He is talking about his other personality, the one that killed her
Then it quotes what the killer said to her before she murdered her, and the narrator/boyfriend or husband wouldn't have known what the murder said, unless he was there. If he was there, he would've saved her right? Yes, so while he was there that "side" of him so to speak wasnt, because the other personality was present.
