Pretty obvious meaning, if you ask me. Of course, everyone has their own interpretations. There is no "true" meaning to a song. What ever it means to you IS the true meaning. (:
"I never said I'd lie and wait forever
If I died, we'd be together
I can't always just forget her, but she could try"
He's not going to wait, knowing they can't be together now, so he's thinking of suicide to possibly be with her, and stop all the pain that the ghost of her, or memories, are causing. He can't just forget about it all, and her. It's not that easy. But maybe she could at least try now.
"At the end of the world
Or the last thing I see
You are never coming home, never coming home
Could I?
Should I?"
He knows he's not getting her back, so it might as well be the end of the world to him.. And this might be him questioning suicide again. Just more expressively. & It sounds to me like he's asking "Could I be with her if I did?" and then "Should I do it?" If that is the case.
"And all the things that you never ever told me
And all the smiles that are ever ever"
It hurts to think of the things that could have possibly been. Maybe they never had a chance to say/do/be what they wanted with eachother, or at least not to him.
"Ever..
get the feeling that you're never all alone?
And I remember now
At the top of my lungs, in my arms she dies.. she dies!"
Although she is gone, maybe he still has some kind of connection with her.. The ghost of her(memories), as I'm guessing the song suggests, or his love for her. But it's still very painful to be reminded of, or remembering. I think it's expressing how much with "At the top of my lungs, in my arms she dies".
"And all the wounds that are ever gonna' scar me
For all the ghosts that are never gonna' catch me"
If I fall..
If I fall.. (Down)"
He has been, and will continue to be hurt by it for a while. He's scarred. Either way, he will try his best to go on though. But he's still not sure if he will be able to.
I must disagree with you but like you said everyone has their own interpretations. If you take a closer look at the name of the song and the lyrics, IMO he is talking about a girl he loved who died. I believe you are right about the suicide, because by killing himself, it will allow him to be with her again on the other side (if there is one). It is even given away in this part...

she dies in his arms and at this part it is him remembering that feeling that she is still with him in spirit or as a "ghost" which the song is so aptly named after all. If you think of it from my perspective, the rest is self explanatory in a "she is dead" point of view.
