I've always thought that, in this song, the guy murdured his girlfriend and is now she's haunting him. Also, I've assumed that he was having a string of affairs with his new wife and has a history of killing the women he was close to. It also gives the repeated phrase 'never again' a whole new meaning, as in she will literally never again be able to do those things. Take the second verse:

(Saying that if she is still with him and knows he's killed and about his string of women, she must have a lot of faith in him)

(Implying that she thinks that their relationship is little more than that)
