I think the singer is directing this song to someone he loves dearly who is struggling with a huge problem (mental illness, self-esteem issues, drugs, whatever). He's watching her give up and urging her to be strong, even though he is starting to doubt their future, and combating the lies she tells herself "I'll hold you close and show you you're not broken." That line just gets me. SWS is phenomenal.
