I'd say have a listen to it again - that last bit sounds a little different to me than what's written here - specifically,

We're far from anything that feels like home", then:
"Here I'm anyone I wanna be
(It's here and now, now it's only you and me!)"
If you listen and hear that, the entire meaning of the song changes. I would say it's about him imagining still being with a lover who was driven away by a mental illness ("It's not what I was told..." a little further on suggests he's in counselling or such). And that would fit with the album's apparent theme of that.
