Actually, I have to disagree with the drug addiction theory. I read the lyrics and it sounds more like just letting go of the past to find redemption. The screaming is about destruction he's brought about, which later wracks him with guilt. But, there's somebody telling him to just let it go while he lays on his death bed.

It has become my favorite song because it reminds me of something that happened to me where I had to end a very good friendship with someone dear to me and it hurt so much.
