Cain is referring to Cain and Abel and to Cain who was the first person to commit murder in the bible. Classic Glenn Danzig who was also a founding member of the Misfits and considered a founder of the "horror" subgenre of music.
I think Twist of cain is about a feeling you get before you kill someone, a feeling in your heart that has been in the human psyche since cain killed his brother.
