This is what I get understand the song to mean.

"It's in this wake that I find myself Losing the will to resume this hell When every breath is a dying wish It's harder to follow the point of this" These lines seem to be contemplating the point of living.

"This broken place that I call my home Is deep in the sorrow that I have sewn" Deep in the sorrow that I have sewn, referring to the mess this person has made in their life.

"And i can't erase what is in my heart I wanted to finish before it starts" This person feels hopeless, and feels like they can't change the way they feel.

"My own solution insufficent again No false illusion, this devouring threat" Trying to fix the problems on his/her own has gotten them nowhere.

"I break the vessel, giving air to it's red and open my fate to the darkened sky" I break the vessel, giving air to it's red, talking about Harming his/herself in some way, and open my fate to the darkened sky, is talking about dealing with the consequences that come with those actions.

"I've been fading away I've been waiting for a call to reach my veins" This person seems to want help, but isn't receiving it.

"No attention to waist Every sorrow of the soul will reap my name When I cease to exist" This person isn't feeling what he/she feels, or doing what he/she is doing for attention, but rather because they truly do feel this way, and "every sorrow of the soul will reap my name when I cease to exist" seems to be saying something like they're going to miss me when I'm gone. I'm a little fuzzy on that verse.

"Now I've come to a final sleep" Seems to be talking about when the person dies.

"I pray that forgiveness is mine to keep" They hope they will receive forgiveness for the things they've done.

"I know it's hard to forgive this man Driving the failure into your hands" Not exactly sure what this line is referring to. Possibly Christ when he was on the cross, and by us doing things that aren't right or whatever, it pains Jesus. Just a guess. Pretty sure I'm wrong though.
