"She only looks like you when it's using me" to me means she's not the same when she's fucked up and "it" refers to her body but it's not her. He also throws in the insulin (used by needle), silver spoon, and blue skin references, uses the word "vein" conveniantly, and I believe the "juggernaut" is heroin as well.
All that aside, I love listening to him singing "and I fear that I'm losing you", cuz it's just powerful the way he says it, and you can lose people in so many ways. That one line gives me an emotional "rush".
