To me, it's about a relationship. The speaker wanted this relationship to continue forever ("In the beginning, felt like you'd always be there"), and expected a lot out of it. The person being spoken to made a mistake that caused the end of this relationship, and in doing so, lost the affection for the speaker. This is the speaker's lonely plea.
The chorus is the speaker's way of showing the other person's ignorance to his feelings and pain ("I don't know how to say to make you understand"). The second lyric shows that the speaker wants to see the other person's open arms, waiting to accept them.
Of course, this is just how I interpreted this song after a breakup very similar to the one described. Nonpoint could've intended a different meaning entirely. ;P
