i a new thought also just popped into my mind, it is kind of a stretch, but maybe if the entire thing is looked as an OD that one person watches and tries to save the ODer's like with rescue breathing, but at the same time is responsible, and then afterwards the issue becomes unspoken, or almost taboo and this is a way to confront it
