I don't think it's about rape at all. I truely feel this song represents a couple that have broken up after a long relationship. He speaks of being afraid of open water, which I think means he is afraid of entering a new long-term relationship, but bathes in sin often, aka sleeps around trying to fill the void inside.
When it gets into the chorus and he speaks of dressed to kill, I think its where they have finally met up again (for some reason I see them in a car at night) and shes dressed up to show him whats he's missing out on. Her wounds are opening wide - old flame still there and they might be just his size - meaning he probably caused them. Reconfirmed when he says there was always warmth between us..
