But does it mean that much to you", I think John means that the person John is talking about in the song does not or cannot understand/appreciate what John "intends to say - but never does."
I know the feeling of never telling someone what you intend to, perhaps because you're nervous or scared - but its the act of preparing to tell someone something and then eventually keeping it to yourself (I would think that most people have done this.)
So I was saying above that I appreciate that John put that in the song because I think it literalized a strong feeling/idea that I know I've experienced.
