I think porpoise head is his nickname for his dick

Personally I don't see it this way, though. To me this song is about being incapble of loving someone without something to tone down the terror of the being vulnerable. In an odd way this song is akin to the Stooges' Now I wanna be your dog, where the terror of being loved is toned down by contempt and domination, fueled by amphetamines and aggression.
