i think this is about being in a mental hospital. the 'shadow faces' are the pyschiatrists/nurses, 'pretending they know whats best for me' when really, if you are mentally ill, no-one really knows what it fels like except you. 'I've lost all ability to make any choices' echoes the idea that if you are in pyschiatric care, your life is not up to you anymore.
and also, i think the "whispered words" may be a manifestation of schizophrenia or general mental illness. and i think "teetering in purgatory" is sort of like being in the grey area between sane/insane.

I kind of agree with you, although I don't think the song nessecarily has to be in a mental hospital, it could simply be about being stuck in a certain state of mind which has her trapped as though in a mental hospital. I think the song is sung very seriously, but the part about "Yesterday's hymn serves me not..tick tock" seems like someone who is being forced to go to church, and they are saying tick tick, wanting it to be over. tongue in cheek
