The song really isn't about anything in particular. There are references to someone being sick. There's a quote from a "Deep Thoughts" skit from SNL. The chorus came from God knows where. And that's that.
Kurt threw lyrics together from various places and what fit together, he went with. Such is the case for the majority of Nirvana's songs. He said himself that there USUALLY is no meaning behind any of his songs. Although- some songs have deep, personal meaning. THIS song does not have any philosophical lesson to be learned.
Kurt was very sarcastic and had a deep fascination with biology. These overtones can be found throughout their entire catalogue.
Please people: enjoy his randomness and freedom. Don't overanalyze everything to the point that it becomes boring and childish. I don't think he would have wanted that.
Oh, to me this song is about three minutes long. Three minutes of great talent from all three musicians.

The first verse is a play on the sort of twee sentiment "I don't mind if you make me sick", here used to describe being trapped in a negative one-sided relationship and the cycle of self loathing that it causes, but while still keeping yourself somewhat removed from the actual pain.

He uses the metaphor of catching a cold off a lover to explain how he has caught his self loathing, but he still doesn't mind letting her "cough on him again", because he still is in love and isn't fulfilled (pun on full fill from her runny nose in an attempt to refill his lost "yolk").
The chorus is about how the cycle continues - wishful thinking of love "in the someday" keeps him holding on, where the negative aspects make him want to "end it someday". I'm not sure about the second part of each line, but it sounds to me more like "what's the hassle" which would make more sense to me. ("can't wait to meet you there, but I don't care", "my heart is broke but i have some glue" etc)
The second verse continues in similar vein, but from the point of view of the relationship being over. Incidentally I'm pretty sure I hear "Finger plaster cast and horse pills". He sarcastically compares mending a broken finger with a finger plaster cast, to trying to mend a broken heart with ketamine (as if it was that easy). He refills the bottle of pills but it's not working because really she is the one he wants to "refill" (sex or possibly also a reference to the earlier "full fill" where he want's to give her back all the shit she filled him up with when they were together).
The quirky cliched phrase line is a reference to the surreal imagery throughout. This and the quote at the end is really to show that the tone is really just him ironically laughing at himself. Or trying to convince himself that he should laugh at himself. Ha ha ha.

And I hate it when some bitch says that kurt was nothing but an asshole that killed himself. KUrt was, and still is, my greatest influence in rock. All this new crap they play on the radio really pisses me off and the nu-metal has become really stagnant. The only "nu-metal" band I like is System of a Down because they original, none of their songs sound similiar and are very unique, and they talk about real problems in the world. Kurt (in my mind) is the greatest artist whoever lived and I can't stand it when someone talks about how suicidal he was. So I try and try and try to get ppl to listen. But I guess ignorance is bliss and ppl are too lazy now a days to listen; even when what they're being told makes sense. They don't even give it a try and look at the facts! =/

You know what I think. I think Coutney killed him, indirectly. First she wanted a baby, so Kurt wouldn't leave her, and later she wanted to divorce. She knew about his parents divorce and how much it hurted him. Actually, his whole life and the music are influenced by it. I also think she used him for the success of her own carreer. i know she met him a long time before nirvana was so big, but WHEN it was, she took advantage of it. She got Frances in the same time when Nirvana went famous... I know Kurt was depressed his whole life and also believe it was suicide, but it was Courtney who made him even more depressed by getting a child and than telling him she wanted to divorce. I know he had a lot of trouble, also whit drugs and his stomach, and i believe he shot himself trough the head, but after all, it was Courtney who made him a really unhappy person. So what I think is that she did not shot him, but indirectly she is a murder.

This note is directed at DEVILDOG Yeah I've seen his suicide note. I agree that the last few lines look disturbingly different, but consider this what if he shot up before he finished it? Here's another question do you think Kurt normally shot up in the greenhouse above his garage when he had his whole huge house to himself at the time? Why would he have gone to the Greenhouse to shoot up when there wasn't even anyone around. Courtney and Frances were in Cali. Another thing is that the door was locked from the inside. Why would kurt have run away from a rehab clinic in the days prior to his death if he was planning on dramatically changing his life by quitting the band and divorcing courtney seems like kicking heroin would have been the first step. Also it's known that courtney was fucking billy corgain while she was still married to kurt. She was with corgain when kurt overdosed in Rome(accident??).I agree that the suicide note is so vauge that it could easily be him quiting Nirvana but either way I don't think we will never know for sure because there will never be a criminal trial against courtney or anyone else for that matter. Either way he was calling it quits and we probobly would never have heard another Kurt Cobain original. It's all just sad as shit and nothings going to chang the fact that he's gone.

This song is completely a joke. Kurt Cobain's last interview was with Rolling Stone magazine on January 27,1994. When the writer asked him, "One of your songs that you cut from In Utero at the last minute was I hate myself and I want to die. How literally did you mean it?" Cobain responded: "As literal as a joke can be. Nothing more than a joke. And that had a bit to do with why we decided to take it off. We knew people wouldn't get it; they'd take it too seriously. It was totally satirical, making fun of ourselves. I'm thought of as this pissy, complaining, freaked out, schizophrenic who wants to kill himself all the time: 'He isn't satisfied with anything.' And I thought it was a funny title. I wanted it to be the title for the album for along time. but I knew the majority wouldn't get it." There is no deeper meaning. It's a joke.
