This song's sweet, Dio is the king of keeping similar lyrics and structure in a song's verses with subtle alterations that keep them interesting. It's hard to explain but it's a really cool effect.
I remember listening repeatedly while on my vacation in Mexico and the lyrical theme kind of gave the idea that it could about the dangers of buying sex while in an underdeveloped country, but that's just me.
I think this song is a warning from a male perspective about sexual immorality in our culture. Many people think Black Sabbath is a satanic band but if you really examine their lyrics you'll find that they're mostly warnings against satanic evil.

I think some of it is actually about Wendy Dio, his wife now widow, but I have heard conflicting opinions on that.

@the starfish Not when Ronnie James Dio was lead singer. Maybe prior Sabbath did. Heaven and Hell is pure Rock n Roll and the best album Black Sabbath ever recorded.
