I think that it was on VH1 Storytellers that Billy said that it was about his brother.
If I remember correctly, he said that his brother wasn't handicapped but was "different".
To me, Spaceboy is a perfect song—probably the best Pumpkins song—about an unbreakable bond between two brothers. Whenever I hear it I think about my younger brothers and how much I love and miss them.
