The song is talking about a relationship that fell part due to the guy's behaviour ( his insecurities ). It's about how the guy regrets what hes done and how hes feeling now because he let her go. He realises the consequences of his actions.

The girl felt as though she was never enough for him , they were both suffering but they still held on to each other despite the pain.

Due to his pain and depression he made her feel like she was the enemy, he hates it, he also hates how she was the one hurt and how he hurt her. He realises how much he needs her now and he hates it. He loved her more then he realised.

he sees them as notes on a page, he is talking about how lonely they are, He is talking about how bad it hurts, its unbearable for him.

He keeps seeing her, he doesn't want to close his eyes and stop her, the memories of her haunt him, he keeps thinking about her basically.

He realises how she felt now, when kept she telling him how much pain she is going through, he talks about how it needed to fall apart for it work - probably because he took her for granted and he realised it when she left and he realised how he felt for her. He sees he is nothing without her. He wants to plead insanity because he was crazy for letting her go.
