This song gives me chills! Reminds me of that feeling of depression and deep coldness that comes with winter.
I think this song chronicles some form of drug addiction (most likely heroine), I've been thinking about this meaning alot..
First of all, the "package" he's buried is the next lot of heroine, stashed away in some random place to hide it. He'd therefore be "a shit in [their] cut" because they won't be able to get their hit they need so bad.
"Pacing in your clumsy ballet" possibly due to withdrawal? He's not sure this is "even your place", showing that the addict(s) are squatting in a random house.
"When the doctor finally showed up" The doctor is probably the dealer, who's arrival they'd been agonisingly waiting for. He has "fur" either because they're trippin' at the time, or they're comparing him to a wolf. Also, when he says "this should do the trick" shows he's obviously trying to give them heroine, as he can't be a real doctor ("we hadn't told him what the problem was").
The "insects he sent off" are his fellow addicts, who he gets to "empty their pockets" in order for him to support their addiction... either that or he's essentially taking advantage of them.
And of course, the recurring "I guess we'll ride this winter out" just shows their mindsets, of just getting high to shelter from the harshness (winter) of real life. I understand that all this is open to interpretation, so I'm probably not right, it's just this seems to make sense to me when I hear the song, especially after listening to the destructive nature of "lampshades on fire".
This was one off the new album that I least liked until I saw him play it live, he opened the show with it and although not the best opener in the MM catalog for a concert, it’s what I remember most. Made me feel comforted with a collective solitude and longing. Kinda a big part of the song, duality in knowing you need help but the only help is that everyone needs to go, THEY WILL ALWAYS BE THE MOST INTELLIGENT CREATIVE BAD ASS MUSICIANS THIS SHITTY WORLD HAS....

When he talks about the doctor's fur being wet, along with the crow sounds, it made me think of the plague doctors from Europe (you know them if you played assassin's creed). They always wore crow masks to counter the bad smells that they thought caused the plague.
