"Dark Side of the Spoon" is a both funny & sad title for an album made by a musical genius who was losing the plot; and this song is a message to his fans & friends saying he knows it. It's painful to listen to so I'm glad the "Keith Richards of industrial metals" wised up and cleaned up. Well done sir.

This is a hauntingly beautiful song about introspection, specifically about looking back at a relationship that started bad and ended so poorly, that the narrator wants to go back to the very beginning and tell himself to not even travel down that road. I believe that the relationship started poorly because of the lines:
"Take me back to the night we met:When the night was full of terrors: And your eyes were filled with tears: When you had not touched me yet"
So, the first night was not a great start, but the narrator pursued the relationship and eventually both overcame the rough start to fall in love with each other:

Like many relationships that turn sour, it was not a quick decline, but a gradual one where the narrator and their partner fall out of love and gradually grow apart

I actually did some research because this is one of my favorite songs. In the beginning, it says "she gave me all her pills, now my back hurts." Due to my research and my own interperetation of this, i believe she gave him the medication Ecstacy. This drug makes people hilucinate and feel as though they are in love, in tune with their surroundings, and be very friendly. Kind of like a "love potion". Ecstacy also leads to dehydration and back pain, due to loss of spinal fluids. This may be where he mentioned "now my back hurts". That, along with previous comments, helps to really interpretate this song. =]
"With a nigga like me" - In reference to the first part "don't fall in love"
"She said please mercy me let me fall out of love before you fuck her" - she fell in love even though he warned her not to, & now he's about to move on to another girl.
"She gave me all of her pills, now her back hurts" - ecstasy: made him feel intense sexual feelings and now HER back hurts (lol).
"Now she pleading, she on the floor" - she's begging him on her knees don't leave.
"She said it won't be long before she falls out of love" - she's trying to convince him to stay a little longer by saying she'll fall out of love soon.
"Just a bird tried to kiss you but you never let me miss you" - Birds come and go never stand in one place, he tried to kiss/miss her but she is always around him/bothering him that he never has time to say he misses her.
"I thought I told you I'm not him" - He told her he's not her Prince Charming, not the guy she wants.
"Nobody forced your hand" - Nobody forced her to fall in love with him.
"Don't you fall for a nigga like me I begged, I begged" - In part 1 he begged her not to fall for him.
"Sandpaper kisses papercut bliss, don't know what this is but it all leads to this, you're gonna leave her, you have deceived her, just a bird (just a bird" - Sandpaper kisses and papercuts are “rough” as are breakups. This appears to be a pessimistic approach to how relationships are: they are bound to not work and he’s going to leave her. The last line states that he’s just a bird in the sense that he will just fly elsewhere and leave her by herself.
I think in this song, when he says "If she only knew", he means that he actually does care for her but at the end of the day he's just a "bird" and he kept warning her anyways because he knew that was what it was. Hearing part 1 and part 2, it sounds like part 1 is a combat song where he wars between the girl and her telling him that he'll fall for her and him denying it. Part 2, its like he won, or he at least pretended to win and now its sad and she's crying and he's just shrugging his shoulders like "I told you. I warned you..."
