i'm pretty sure this song is about how she fell in love with a boy who was 'crazy' or had some mental illness, and didn't have any faith that their relationship could work because he may have thought that nobody could love a crazy person. "you're a trainwreck but i wouldn't love you if you changed" says that she loves him even if he is crazy, and if he wasn't he wouldn't be himself.

it's weird. my ex is way more insane than i am, i think people have always thought i was more insane than i am because of the bullshit that comes out of his mouth, and no one ever tried to send me to a doctor or give me pills because i have anxiety. i had to do that on my own. i think the reason i was a trainwreck was because he made me have nervous breakdowns a lot.
