i think this song is about being betrayed and trying to forget it. "knife going in"=backstbbing i dunno love this song though

Well if she said it was about the death of her grandma... maybe the Knife Going In is like symbolism for what the feeling was when her grandma died. Like a knife going in, but not killing her. And her reaction was to run, hide, get away from the feeling. Asking Emy, her girlfriend, if she should stop worrying and fretting. Whether she would make it to the next morning without her grandma. She wants emy to stitch up the blow of her grandma dying. And she knows shes writing a song that'll make her mother, her sister, and her lover worry about her.
After reading all the comments above it made me decide this song really is a lot more simple.
I think it is about a relative passing and then feeling/being suicidal.

"Not Forever // On the night I die // I swear I'll sleep outside your window" Being away from each other isn't going to last forever. Most would liked to believe their loved ones stay and watch over them.
"I feel the knife going in // I'm feeling anxious // Not enough to kill me // I thought it'd happen fast" I don't know why this didn't ring suicide or cutting to anyone.

That's the funny thing about this site, sometimes it just really isn't anything deeper than what it says.
I think it would help you understand better if you could just relate to the song and really feel the meaning.
Otherwise you really would need to do all the backstreet boy background check... what's their favorite food, what color are their underwear, who's their daddy.
Just hearing her voice in how she sings this song, the pain should translate it perfectly. Especially how haunting it is when she sings "I let it pass". Even just how the music sounds music box almost. Just sets an eerie tone for their angry/calm singing and it shows how contemplative the lyrics are.

That said, I've not much of a clue as to what this song means. "She" probably represents something intangible rather than a person.
