This song is about a "lonely" person, somebody who has (mentally) abandoned all the people around him/her and lives a life of (temporary) solitude..."Dr. Strangeluv"=he/she doesn't connect/love or allow himself/herself to connect/love on a personal level to anybody (yet)
It is about somebody who has had the realization that most of the people around him/her are shallow, conformists ("looking everywhere, I see nothing but people...cold look in their eyes, cold air in their hearts")
He/she used to think that he/she belonged with these people, that he/she needed these people in his/her life ("I always thought I could never leave them")
These people are calling his/her name, they want him/her to be with them, to be apart of them, but they don't know that he/she can't keep up this fabrication for very long, when forced to at times (at work, "how are you's", proper etiquette,. etc.) ("They are calling me, but they don't know I can't stay all night long")

great explanation trust_no1 and often times i find myself in that situations and feel like im useless even though i tell myself that i could make ppl to be better or what not but it aint easy job...
so i decided to close my mind rather than to share my mind with others and trying to make a differences

as good as your projection onto this song may fit, im gonna say its about nuclear war. ha just joshin. really all i got to say is that it sounds like a dark night of the soul to me, which i think fits with the whole 23 magick concept of the album.
This song is about Misanthropy and Solitude.
I don't think this is about misanthropy. I think there's deeply imbedded in the song a feeling of appreciation for being around others, but maybe from some distance, but I don't think that distance is intentional, it's more circumstantial, like something inevitable. And this what I think this is about. It's about relationships and how the fluctuate over time. I was watching the other day a video on YouTube where Kazu talks about the making of this song, and I am unsure if I understood correctly, but it seemed to me that she said that this song was about her fans, her colleagues, her friends, in general, the people she has interacted with and that she wonders who's gonna stay after all, who's really there for her. "Looking everywhere I see nothing but people. Where have they gone? I always thought I could never leave them. They are calling me, they don't know I can't stay all night long". I don't know how Dr, Strangeluv fits into all of this, maybe it's just sort of a wordplay of the words strange and love and not referencing a specific person or character. But indeed, this is so sad, it is true.
I love that line about how his/her friends are calling but that the person can't stay "all night long." The metaphor of the sun setting on those relationships is beautiful. To me the song is about some relationships deteriorating because people have gone cold or they have lost themselves, or they are no longer good for you, or all of the above.. and that eventually ends in the protagonist doing what is right for them and leaving.
The lines "I always thought I could never leave them" "I always thought that I could save them all" implies that the lost friends are in need of saving and the protagonist really cares about them, but the protagonist realizes that although it's "So sad, so bad" they can do nothing to save them and that is why he/she has to leave...
Keeping my fire burning is used to juxtapose the warm and light condition of the heart and eyes of the protagonist while the heart and eyes of his/her previous friends have gone cold.
the contemplative "isn't it true" seems to be the protagonist questioning although leaving was good and necessary they question how much it hurts.
I'm sure there is a lot of personal interpretation in there. To me this song is beautiful.
