It really is about having a cold on the beach. Sampintoknowsitall is serious. Hence the, "In spite of sunburn, I had a head cold I blew my brains out through my nose"

That makes sense, since the first line "Trade your demise for mine A tissue for a cigarette" is saying, "I have a cold, and you smoke, so I'll trade my tissue for your cigarette."

i think he means he'll give the guy a cigarette if the guy gives him a tissue

i think he does drugs through his nose, "i blew my brains out through my nose" ... just thought the best song/reason the cd was named prolly wasnt about a cold
