Otep dreams of a place, "this brutal gutter" I'm not sure if this is a place she longs to be, but she dreamed of it and then she awoke. Simple as that.

"This brutal gutter" isn't where she longs to be...pay close attention to the lyrics again. She's talking about dreaming of a better place without anything from this world. "This brutal gutter" IS this world. But she wakes up and realizes that she just can't escape the real world and has to face it.
