this is a very beautiful song, the only horrible thing about it is how short the damn thing is.

I've never heard this one, but I like the lyrics.
I think it's about someone who lives in a city and misses someone they were with and who has left them for whatever reason, so now they fell lonely.
"Looking out the window At another window I see toenails changing colours Like the leaves of fall" This indicates they live in a city - instead of seeing trees changing colours, they see other houses and people who paint their toenails different colurs now and then.

I think it's because the piano sounds like a lulliby that you would play to a child, very calm and soothing.
I think the song is about someone who lost someone they love, but they both still care about each other.
The line "If you often smile but often don't smile Which do you do more often Smile or not?" shows the uncertainty of the other person in the relationship as to whether or not they want to be in the relationship.
I always interpreted the toenails changing colour as a sign of aging and time passing by, because I've noticed the older people are the yellower their toenails appear. I do find the citylife/nail polish interpretation interesting as well.
I think this is just a rumination on 'the one that got away' - how s/he can't forget him/her even as time/seasons pass:
