fuckin' scary song. rightly so, given the title.
I fucking love this song. To me, it's about not wanting a relationship to end when the other person in it does.
I don't think this song is about a girlfriend or a girl in general. I think it's a personification of a girl's addiction. "Her friend's a bitch and needs to go" is referring to ciggarettes in the first part and alcohol in the second part. Even the verses saying "I'm in the house" he is saying nothing will get her to stop, not a new boyfriend, not a kid and definitely not a piece of paper. He hates that she is unwilling to stop these vices because they are killing her and making her look careless and he tries to help her but she never appreciates him or accepts his help.
This is one of those songs where I wonder how everyone else is getting the interpretations I keep hearing.
To me it seems, pretty obviously, to be about a mother (possible Sage's) and addiction (cigarettes and alcohol), and her bringing men home because she thinks her kids need a role model (which, if this song is about Sage's childhood, they finally got with Sage's stepfather). The song might not be so personal, but I think the addict mother interpretation makes a lot more sense than relationships ending, or a girlfriend, or any of the other interpretations I've seen. I think KeepSubjective is right about the personification of her addiction as "her friend" (and the speaker in the chorus)

I think this holds it... obviously Sage Francis doesn't like drugs, he's a well known straight edger. I think it's a point of, from the perspective of someone who doesn't do drugs in a subculture (hip-hop) rife in drugs, I've seen what addictions do to people and how it ruins them.
The part at the end with "Fuck you! Get the fuck out of my house!" is dialogue from the movie Magnolia.
