It's about addiction.
The title stands for "If you don't kill me I'm gonna have to kill you". The idea being that if the addiction doesn't kill the person, they're going to have to kill the addiction.
The "it referred to is the addiction. "It's been doing some thinking. . ." "whether it was born or bred, genetic, environment" i.e. was the person born with a personality to be addicted to alcohol of drugs or was it acquired through the environment?
"So longs to hear her final words, I brought this on myself". Addiction leads to death- a favourite Type O theme.
Then quite possibly my favourite lyric ever "The theory of futility, now you learn, so be prepared- Enemies are equal to wrath times the speed of fright squared". It's a parody of Einstein's theory of relativity- Energies are equal to mass times the speed of light squared- (E=mc^2). You can't run away from an enemy that is a part of you- it's effectively a universal constant.
Department of Probation, legally it can't be blamed- you can't lock up addiction- it's just madness.
The "Gimme that" is what the addiction makes people say. The outro "no place to hide, your choice not mine" is what confronts every addict.
I think the track's very cool. While addiction from the drug's point of view has been done before (Master of Puppets), not many people have Pete's turn of phrase, or cynical humour.

this is my favorite TON song. I listen to it when i work out and always lift better. when pete says "IYDKMIGHTKY" I am always looking in the mirrors at the gym cause im a self loathing asshole.
I think the song is about a psycopath stalker who is after a girl he obsesses over - "it lies awake yes quite obsessed." Obviously the killer must be killed to be stopped; hence, "IYDKMIGHTKY."
I think he ends up killing her, and goes to trial for his crimes - "dep't of probation" - but since he is insane he doesnt go to jail, "what a shame".... haha

I subscribe to that idea: a mentally unstable person killing a girl. Also, there seems to be an idea of treating the mentally unstable person as an object and not a human, thus the pronoun 'it' and not 'he'. Insanity seems to be a theme in at least one other Type O Negative song: Who Will Save The Sane off World Coming Down.

^^^I'm the same way - I listen to this a lot while working out, I get a feeling of "killing my current self" and replacing it, to be overly introspective and pompous.
The "Gimme That" thing is just the usual Type O goofiness to go alongside the morbid stuff.
The theory of futility part is funny.
if you dont kill me then im going to have to kill you, an acronym [IYDKMTIGTHTKY] and a metaphor. imo it stands for cause and effect, anything you do [even if its nothing] has some sort of outcome. "you must decide, your choice not mine"
to me, as a big type o fan for over ten years now, i think that this song is about doing something that you regret, but you're glad you did it. does this make sense? being so frustated with someone/something that if it doesn't kill you, you will have to kill it.

This is an updated version of 'Der Untermensch.' Same idea, of the sub-human social parasite (Gimme dat, infers an alien parasite) How the scum keeps building up in society, and people's (by & large) 'whatever is trendy now' moral code. He's trying to turn us listeners into conduits for positive, effective, fact-based (over the prevaling emotion-based) change. I mean by that, the 'bad guys' get society to accept detrimental changes to their environment, by appealling to your (missplaced) empathy, for a creature that would (without hesitation!) stab you in the back for your tennis shoes. The media has made dupes out of everyone, except the few who are to be labeled as 'insane.' Effectively nullifying our FACT-based arguments.
