I think the song is about a psycopath stalker who is after a girl he obsesses over - "it lies awake yes quite obsessed." Obviously the killer must be killed to be stopped; hence, "IYDKMIGHTKY."
I think he ends up killing her, and goes to trial for his crimes - "dep't of probation" - but since he is insane he doesnt go to jail, "what a shame".... haha
idk what the "gimme that" is about... perhaps sex.

Also, there seems to be an idea of treating the mentally unstable person as an object and not a human, thus the pronoun 'it' and not 'he'. Insanity seems to be a theme in at least one other Type O Negative song: Who Will Save The Sane off World Coming Down.
