I think he know that he is going to die and the doctors and hospitals can do nothing to help him. he probably thinks that the life he has lived is pointless and that he never achieved anything "if you would tell me just what my life means. walking a lon road, never reaching the end."
When he sits alone in his room and gazes out on the rain with no purpouse whatsoever he cant see a maning in his life and he knows he will die a pointless depessed death.
the man is probably religious and does not understand why god lets him die such a meaningless death. is that the way god treats his followers? left alone on his own with a deadly desease and nothing but melancholy left.
at last he starts wondering if he has been as good and faithfull servant of god as he should have been. he has lied and sinned but noone culd expect such a thing from a verry religius man.
in the end of the song he cries out to god for answers for all his questions.
this song has a deep melancholy and a subreligius theme. the instruments are played very nicely and the riff and notes are verry apropriate for the themes of the song. bruce sings this song realy good although it would be fun to hear this song with blazes haevyer and somewhat deeper voice. it is one of the more depressing songs of iron maiden, beaten by "2am" and "como estaeis amigos"
