I think this song is about a person who, from an already bad place in their lives was placed in an even worse one. Although they hate were they are in their lives, it has also a part of them and because they don't know of anything else they not only accept it and but they even go so far as to welcome it. As it says in lines 8 and 31 "My Orphanage, My hateful place.", "My Orphanage, My Dear." However, with that acceptance comes hate. In hating where they come from, they also hate themselves; as do a lot of us, in some from or another.

I also support the bad place in someone's life theory. ..."For longer than my years"... How people become alienated by other people. Maginated, and self-isolation. And how we want out of this, but "this is the only home I know". I guess it just talks about life in general. I guess no one in reality is actually happy. At least most of the time.

Like that case, this place I carry / Inside of me. It's not so very / Heavy for a stocky child.

I think that line might mean that when you grow up in a certain place, it doesn't seem as bad because you become used to it, and even though you know it's not ideal while you're there it seems better as you move on into worse things. [or merely different things, for isn't the world just as bad as an orphanage? but she was used to the orphanage.] eh, I'm probably not making sense right now.

I think the phrase "My Orphanage, My Heart" means that even though she does not enjoy "My Orphanage" she can't kill it, she can't leave it, because that's all she has to keep her alive, hence "My Heart"

I believe she is using an orphanage as a metaphor for her heart. Thus, "my orphanage, my heart." She presents the orphanage to be a tangible thing or something she is experiencing (where she lives) until the very end, where she tosses out "my heart" and makes you rethink your perception of the meaning of the lyrics.
