I think Hilary Duff's trying to show us in her lyrics, 2 different things.
1: She had a break up with her boyfriend and she's expressing how her heart is broken.
Or
2: The chorus is telling us about her late teenage life when she was going through harmful body issues and she became really thin. For proof, you can watch her AOL session singing 'Break My Heart' and she looked extremely thin, didn't she? It's like she's saying that there is somebody who she was really close to, but then whatever made her feel the way she felt COULD be the translation behind this chorus. It's like when she was saying in the chorus 'watch me fall you see, I'm falling apart, look what you're doing to me'. See how I mean when I say it's like she could be expressing that?
