Though I think the other comments have truth about the loneliness, maybe this is just how I see it because it’s something I face as I struggle with bipolar, but I believe she is implying that either she or whoever’s perspective she’s singing from struggles with emotional regulation and uses vices to drown the pain of her self inflicted loneliness, the guilt of her volatile behavior, and the feeling that she will always ruin the relationships that mean the most to her because mental illness will often make you feel like there is something missing inside. Again, just a personal perspective.
