I think, and forgive me if I'm wrong, that certain lines throughout the song imply an apocalyptic event. And some other lines kind of sound like someone coming from a life of poverty or working class talking to another individual of a higher class, such as royalty or a more privileged life, who is denying the fact that the world's situation is taking away some of their luxury and social status, as in an apocalypse, none of these things matter. Also, the name of the song can be interpreted as being in n a situation where you're in an area where mass death or disease struck, and you can't leave. I also find the song has a certain feel of urgency, or danger, or crisis of some form, which strengthens my theory of apocalypse. I could be wrong, though, as this is just a theory. I am curious to hear some other opinions and theories as well.
